💳 Payment Policy

• Payment is due upon receipt of your invoice. You will receive a message through our EMR system with instructions and payment options.

• Accepted Payment Methods:

- Venmo: @Theresa-McDonough-1

- Zelle: 716-310-7094

- Credit/Debit Card: via link provided in your EMR message

🔐 Credit Card on File

• A valid credit card is required for all new and existing patients.

• Your card will only be charged if:

- A payment becomes more than 14 days overdue

- You fail to respond to repeated payment requests

💸 Payment Plan Options

• We offer monthly payment plans, as well as biweekly or weekly plans upon request.

• Payment arrangements must be made in advance—please email us to set up your preferred schedule before your balance becomes past due.

🕒 Late Payment Policy

• More than 5 days late: A $25 late fee will be applied to your balance.

• More than 14 days late: A $50 late fee will be applied, and your card on file will be charged for the full outstanding balance.

💊 Medication & Refill Policy

• Medications and refills will be ordered only after payment is received in full.

• By agreeing to home delivery, you commit to paying for all medication shipped to your address.

• If a shipment is damaged or does not arrive, it is your responsibility to notify us immediately at [email protected].
